Information About Your Appointment
- Minors (17 years and younger) must be accompanied by a parent or legal guardian
- Insurances vary. Please check with your insurance regarding covered benefits and
provider participation
- Please bring your insurance card(s) and drivers license or picture I.D.
- Worker's Compensation appointments will NOT be scheduled until authorization has been
received at our office from the workman's compensation carrier in writing. We cannot
accept a verbal authorization.
- If this appointment is due to an auto injury you will need to hand carry your authorization
including: name and address of insurance company, phone number and claim number
- HAND CARRY all films you have from previous X-ray, MRI, etc. Your physicians office will
not forward films to our office. If you have not had previous X-rays we have facilities here
to take them
- We suggest you wear loose-fitting clothing in the area you are being seen for. Disposable
shorts and gowns are available as well
- Actual appointment times will vary depending on the complexity of the problem. New
patient appointments could be up to three hours by the time records are reviewed, xrays
taken (if necessary), follow-up tests and/or surgery scheduled (if necessary),etc.
- Your appointment is to meet with the orthopedic surgeon, however, surgery is not the only
option. Other alternatives include injection, physical therapy, etc.
- Please be aware: you may be asked to reschedule your appointment if you do not
have with you at the time of your appointment your insurance card(s), driver's license,
X-rays, CT scans or MRI films and relevant test result documents.
