Questions
Do I need a referral
to be seen? A
physician referral is not required to make an appointment. However,
you should be aware that some insurance companies require a referral
from your primary care physician before they will authorize payment
for the office visit. We recommend that you contact your primary
care physician or insurance company before your scheduled visit if
have any questions.
Where do the physicians
at ELOA do surgery? The physicians
do surgeries at Ingham Regional Medical Center, St. Lawrence Campus,
Sparrow Hospital, and Genesis Surgical
Center.
Do you participate
with my insurance? We
participate with Medicare, Blue Cross/Blue Shield, Physicians Health
Plan (PHP),
Blue Care Network, and Sparrow
Physicians Health Network. We do accept most other insurances, but due
to many variables in policies we urge you to check with your insurance
company prior to any treatment.
What do I need to
bring to my first appointment? Your
insurance cards or written authorization from the carrier if your problem
is work related or from an
auto accident. You will also need your driver’s license or other
photo I.D. Please bring any x-ray, MRI, CT or other studies related to
the problem for which you are being seen.
How should I dress? It would be helpful to wear loose clothing in the area to be treated, but we have gowns and shorts available.
How can I get my medical
records and X-rays? Records
are held in strict confidence. We do require a written request from
you authorizing the
release of your records. We do require 24 to 48 hours for processing
your release.
My son/daughter is 17 years old. Can they come to their appointment
without an adult? No, we do require minors to be accompanied
by an adult.
I have neck, back
or rib pain. Will you see me? We
do not treat these areas. We recommend you see your primary care
physician, who can
make the appropriate referral.
What is HIPAA? Health Insurance Portability
and Accountability Act of 1996. The Act was passed to protect
you,
the patient. We have
a policy
in place dedicated to maintaining your privacy. The policy
is available at the office for your review.
